REGISTRATION FORM
 
You can register online or bring in the enrollment forms to the front office. Enrollment Forms will need to be completed upon online registration.
What is needed by all students upon enrollment:
  • Completed Enrollment Form | Download Form
  • Birth Certificate
  • Current Immunization Record
  • Last Report Card (non-Kindergarten applicants only)
  • Proof of Residency (utility bill, lease agreement, mortgage bill with address on it)
   
Registration does not guarantee placement. Placement is dependent upon space available.
